I created a personalized tour plan using our fast passes. Our trip is about 50 days away. Do I need to go back and optimize again when it gets closer to my trip?
It would be a good idea to re-optimize or at least re-evaluate your TPs as you get closer to the date of your trip. Most Liners even recommend doing it again as late as the day before your park visit because things can change.

Yes - the TP researchers often update their wait time predictions close to the actual date in question, so it is always good to re-optimize or re-evaluate your plans so that they are based on the latest data.
