Is it possible to add attractions from IOA and USF in the same day’s TP? Or do I have to do a morning one for one park and a separate afternoon one for the other park? We have park-to-park passes.

No. Just like WDW, one touring plan can only be for one park.
TP has some pre-made one day/two park plans that will give you an example. They are part 1 and part 2 plans.
