I’m looking at the menu and have a couple of questions…
Regarding The “Plaza” Ice Cream Sundae, will they do that NOT in a waffle bowl?
OR
For the All American Ice Cream Sundae, will they substitute caramel for the peanut butter?

The Disney restaurants are always really good about working with guest with their special requests. My DW is really picky when it comes to food. She asked the CM at the Plaza to make a substitution on one of the toppings of the ice cream item she ordered, and they were happy to accommodate her. I don’t think you should have any problems with any reasonable requests like yours.
